The Health Action Centre is a WRHA Community Health Centre located in the downtown area providing comprehensive health services to the members of the surrounding community. The Health Action Centre is a member of the Winnipeg Regional Health Authority.

Primary Health Clinic
Providing total family health care for all your health problems.

•Primary Care: Doctors and nurses are on site to provide regular medical care to help you and your family stay healthy.
•Pregnancy Care: Your doctor and nurse will ensure that your pregnancy and delivery go as well as possible. Midwives can also be consulted.
•Well Child Care: Baby check-ups; immunizations; nutritional information. All are available to help keep your child healthy.
•Reproductive Health: Our reproductive health nurse provides individual and group counselling on sex and sexuality, birth control, pregnancy and sexually transmitted infections (VD).
•Nutritional Counselling:Our dietitians can provide information on healthy eating and special diets.
•Dental Services: A dental office located in the clinic provides affordable dental services to the community.

Outreach Services
Reaching out to help people to help themselves.

•Social Work: Our social worker can help you with counselling, assessment and referrals.
•Community Health Workers: Helping people to access services and solve problems.
•Meal Program: Provides a nutritious hot meal in a friendly environment to members of the local community 5 days a week.

Health Services For the Elderly
Helping our seniors stay healthy and independent.

•Nursing: Our nurses provide assessment, treatment, monitoring and educational services aimed at improving and maintaining health and wellness.
•Nutrition: Our dietician provides one on one and group assessment, education and monitoring of the special nutritional requirements of our elderly clients.
•Therapy: Our physiotherapist and occupational therapist will help improve our clients’ physical fitness, mobility, independence and safety.
•Hygiene: Providing hygiene services to clients with needs than can’t be met at home.
•Foot Care: Our nurses are trained in diabetic and complex foot care.
•Social support: Providing leisure activities in large and small groups to promote an environment of caring and sharing.

Midwifery
Midwifery care includes quality health care, education, counselling and support for childbearing women. The midwife works with each woman and her family to identify their unique physical, social and emotional needs. Under midwifery care, a woman does not need to see a doctor unless there is a medical condition that requires a doctor’s care.

Public Health Services
Providing tools and support for a healthy community.
Public Health nurses and support workers provide a range of services in the community to promote health and wellness to families
•Childbirth Education: Preparing mothers and their families for childbirth and parenting.
•Antenatal Home Care: Care for women experiencing high-risk pregnancy.
•Post Partum Follow-up and Family Support: Support for new mothers and their families.
•Baby First: Supporting new parents to care for their baby.

Home Care
Helping people stay home safer, healthier, longer.
The Manitoba Home Care program provides home support to individuals, regardless of age who require health services or assistance with activities of daily living. Through assessment by a Case Co-ordinator, your eligibility for Home Care services will be determined.

Geriatric Program Assessment Team (GPAT)
Comprehensive Assessments and Creative Solutions for the Challenging needs of Older Adults.
GPAT assess the complex needs of older adults, in their homes or in the hospital, and help to access appropriate geriatric services. Their purpose is to reduce or prevent unnecessary hospital admissions and recommend treatment plans to improve their clients’ quality of life.

The Health Action Centre has Mental Health Workers on site. The Community Mental Health Workers’ roles are to promote mental health in the community and support those who suffer from mental health issues.

Shared Care
What is Shared Care? Shared Care is a mental health program that involves family physicians working with mental health counsellors and psychiatrists. The goal of shared care is to assist persons with mental health difficulties and help them achieve and maintain good mental health and emotional wellbeing.
How does Shared Care work? The Shared Care program is used when a family doctor determines that a person is experiencing mental health difficulties, or when a person indicates to their doctor that they are experiencing mental health difficulties. In these situations, the family doctor may refer the person to a mental health counsellor or a psychiatrist. The doctor, counsellor, and/or the psychiatrist will then work together with the person to identify the support and assistance they need to achieve their health goals.
The doctor, counsellor, and/or psychiatrist work in the same office or clinic, so all services can be offered from the same location.
What are some reasons that a person might be referred to a mental health counsellor or a psychiatrist? There are many reasons that a person may be referred to a mental health counsellor or a psychiatrist, including: • Feelings of depression or sadness • Problems with alcohol and drugs • Social isolation and loneliness • Relationship and parenting problems • Experiences of grief and loss • Feelings of anxiety • Conflict at home • Low self esteem • Difficulty coping with stress • Difficulty at school or work
Many things, including life experiences, ability to cope, financial security, and the availability of social supports, housing, and access to health care, influence a person’s mental health and emotional wellbeing.
Do mental health counsellors and psychiatrists offer the same services? Typically, a mental health counsellor provides individual, family and group counselling that is short-term and time limited. A psychiatrist provides assessment and treatment for those who need specialized health care.
Is the Shared Care program available to everyone? No. At the present time, the Shared Care program is only available to persons whose family doctor works at one of the six WRHA primary health care sites, including the Health Action Centre.
How does a person receive mental health support if their doctor works at a location that doesn’t offer Shared Care? All family doctors have access to the services of mental health counsellors and/or psychiatrists, and can refer their patients when services are required. Doctors who work in Shared Care facilities have added advantage of having counsellors and psychiatrists on-site.

Downtown Seniors Resource Council
Bringing Seniors and Services Together
The Downtown Seniors Resource Council The Health Action Centre has on site a Resource Co-ordinator whose role is to act as a point of access for information and referral, as well as identify and develop new services needed for seniors at the local level. These services may include:
•congregate meals •friendly visitors •cleaning and home maintenance •transportation •safety checks (daily hello) •emergency response programs

Community Connections
Connecting Communities in the Heart of the City
•Central Winnipeg Community Network: The Health Action Centre is one of many Access Sites for the CWCN. A computer is available on site to members of the community enabling them to access the World Wide Web. Volunteers and recourse persons are also available to help people learn the basic skill necessary to use the equipment.

•Web 4 All: The Health Action Centre is also a host site for Web 4 All. This program was developed to ensure seniors, new Canadians, and people with disabilities or literacy challenges have computer access to services and information on the Internet. Come and try it out.
